Safety and security for your family through intervention and prevention.

A street-level walking resource.

The Community Care Team reduces the stressors linked to the high rates of violence and trauma that people of color face. Meeting people where they are is our value proposition. Making connections and building relationships means those we serve get a better chance at stability in their lives.

Healing
Hurt People

Now serving Legacy Emanuel, OHSU, and Legacy Mt. Hood. In-hospital support for victims of violence and their families during traumatic events. HHP Team members advocate for patients and help bridge the gap with hospital staff to help families navigate the resources needed to recover.

Public Safety
Village

The Village consists of 11 grassroots, Black-led organizations led by individuals impacted by Community violence. This collaboration fills regional gaps in violence intervention services by providing cognitive behavioral therapy, conflict resolution, recreation opportunities, youth grief/loss support groups, decision-making skill-building, and more.

Trauma + Violence Family Support

Community Care Team members have lived experience and provide effective mentorship and support after a violent incident. Families receive assurance of having their basic needs met, which aids healing. The multi-pronged approach lifts up the victim's support network, too.
